Important Update to MP Office Passport Procedure
The Privacy Act and the Canadian Charter of Privacy Rights govern Passport Canada and consequently, no communication with MP’s offices can occur with regard to a passport application, including its status within the entitlement determination process, without a written consent by the applicant.
It is also important to mention that all passport applications brought to an MP’s riding office in sealed envelopes must not be opened by the MP or his/her staff.
However, my office remains ready and willing to assist constituents with questions related to their Passports.
Accordingly, if you would like my staff to help you review the application before it is sent to Passport Canada please ensure that it is in an unsealed envelope. Additionally, if you would like my staff to be able to make inquiries on your behalf, which includes status reports on the processing of your Passport, please make sure to include with your application an Information Sharing Consent Form. Please note that an Information Sharing Consent Form must be submitted by each applicant.

I am happy to forward your passport application to the Canadian Passport
Office. Below are some questions frequently asked by my constituents
regarding this service.
Q - How long does it take for my passport to be returned to me when I send
it through your office?
A - Please allow up to 6 weeks for processing. Processing time may vary. The
passport will be sent to your home. You are required to sign upon receipt.
Q - What should I do if I require a passport within a month?
A - You should take your completed application to a passport office in
person. If you have a family or other emergency, the passport office will
require documentary proof from a physician. The passport office will assess
your request for urgent processing and costs, and will advise you of
processing time. Passport office locations are listed on the website.
Q - Where can I obtain a passport application form?
A - Forms are available in most post offices, in my constituency office, and
online at the passport website (http://www.pptc.gc.ca) . Please note that
there are three types of forms: Simplified Renewal Form PPTC 054, Adults
Form (16 years of age or older) PPTC 153, Children (under 16 years of age)
PPTC 155.
Q - What information do I need to complete the form?
A - Instructions for completing the form are enclosed with the application.
Please answer all relevant questions including date of departure if known
and sign in the correct box. The passport office will not accept white-out
for corrections.
Q - What is a guarantor?
Your guarantor must:
1. Hold a five-year Canadian
passport that is valid or has been expired for no more than one year, on the day you submit
your application;
2. Have known you personally for at least two (2) years.
In the case of a child, have known you (parent or legal guardian) personally
for at least two (2) years and have knowledge of the child;
3. Be a Canadian citizen 18 years of age or older;
4. Have been 16 years of age or older when he or she applied for his or
her own passport.
Q - I have just moved to Kingston, and do not have an acceptable guarantor.
How do I meet that requirement?
A - My office staff can supply the form PPT 132 (Statutory Declaration in
Lieu of Guarantor).
Q - What is "original evidence of Canadian Citizenship"?
A - You must send an original Canadian birth certificate or a Canadian
citizenship card. The commemorative certificate is not acceptable.
Q - I was born in the Province of Quebec. Can I use my birth certificate or
baptismal certificate?
A - As announced in Quebec in October, 2001, the passport office will accept
as proof of Canadian Citizenship only those Quebec birth certificates that
were issued after January 1, 1994. If you hold any document issued prior to
that date, you must apply for a new Quebec birth certificate.

Q - What is the cost of an adult passport?
A - A new 24-page passport costs $87. A 48-page passport costs $92. You may
pay the processing fee by certified cheque, money order, or Visa,
MasterCard, or American Express (see the application). Unfortunately, my
office cannot accept cash or debit.
Q - What is the cost of a child's passport?
A - Child under 3: $22 (3 year validity). Child 3 - 15 years: $37 (5 year
validity)
